- Report of Birth Form, notarized (4 originals)
- Original valid passport of child’s mother; and father’s original valid passport, if parents are married or the child will use the father’s surname, plus 4 copies
- PSA-issued Marriage certificate or Report of Marriage of the child’s parents (if applicable), plus 3 copies

Submit application via email with one clear copy each of the documentary requirements listed to civilregistry@philembassy.gr for pre-assessment

The Civil Registry Unit assesses the completeness of the emailed submission
Step 3
Once the emailed documents are determined to be complete and in order, the applicant either submits 4 sets of the documents with the originals personally, by appointment, or mails them to the Embassy

Payment will be made in the form of cash if the applicant appears personally or through bank deposit to the Embassy’s account, if application is by mail. The bank details will be given to the applicant once the application is approved
Step 5
The personal copy of the Report of Birth will be given to the applicant. It may be claimed personally or sent by courier at cost to the applicant.

You may request PSA-authenticated copy of the Report of Birth after 6 months from the date of reporting at https://www.psaserbilis.com.ph/#!
