REPORT OF MARRIAGE

BASIC REQUIREMENTS
  • Report of Marriage Form - notarized (4 originals)
  • Original Valid Passports of both parties, plus 4 copies
  • Original Birth Certificates of husband and wife, plus 4 copies PSA-issued for Filipino spouse; Officially issued by vital records office or government registrar for foreign spouse
  • Data page of passports of husband and wife valid at the time of the marriage (4 copies)

ADDITIONAL REQUIREMENTS

IF REGISTRATION IS MORE THAN ONE YEAR AFTER THE MARRIAGE
  • Affidavit of Delayed Registration of Marriage (1 original and 4 copies)
  • PSA-issued Certificate of Negative Record of Marriage of the couple (1 original and 4 copies)

STEPS

Step 1

Submit application via email with one clear copy each of the documentary requirements listed to civilregistry@philembassy.gr for pre-assessment

The Civil Registry Unit assesses the completeness of the emailed submission

Once the emailed documents are determined to be complete and in order, the applicant either submits 4 sets of the documents with the originals personally, by appointment, or mails them to the Embassy

Payment will be made in the form of cash if the applicant appears personally or through bank deposit to the Embassy’s account, if application is by mail. The bank details will be given to the applicant once the application is approved.

Step 5

The personal copy of the Report of Marriage will be given to the applicant.  It may be claimed personally or sent by courier at cost to the applicant.

You may request PSA-authenticated copy of the Report of Marriage after  6 months from the date of reporting at www.psaserbilis.com.ph.
